MacTheRipper can rip files off the DVD, but I don't know how you would go about adding subtitles? A quick search reveals methods for Windows, but not mac.

If you want to add subtitles manually, you could rip via handbrake, and add via Quicktime Pro/iMovie/FCP, then re-burn.
