| The "Terminating on Signal 15" seems consistent with my logs for a normal log-off. However I can't see the Hangup (SIGHUP) message in my logs, so I'd assume you are being hung up on from the other end.
If you have call waiting on your phone line, make sure you have it turned off, as this might disrupt your modem connection.
