Help! Program Launch Failure

Hi All.

I am a new member and relatively new mac user. Hoping someone out there can help me! I am running OSX on a G4 Dual processor desktop. The problem briefly is this; was running an application until very recently without a problem, then unexpectedly when trying to launch it in the dock, a black question mark appeared over it and it failed to launch. I removed the program and all of its components, trashed them, rebooted the mac and re-installed the program. This unfortunatley did not solve the problem, and every time I try and launch it - nothing happens. It looks like it's trying to launch (the hollow square comes out of the icon and fills the screen, but nada!). At my wits end here. I have a crash log which I hope helps! As follows;

Forgot to add; have run repair permissions on disk utility and fsck on single user mode also, and it flagged no problems. Would really appreciate any help on this.
