Mail attachments and MyFax service

Mail 2.1.3 / MyFax service
Lately the emails received that are a fax sent through the myfax account show the same cover page 3 times when I open the mail.
And now when I go back and look at one of these emails in the Trash - it is showing an attachment from an entirely different email (myfax email) within the message.........
I contacted myfax and they checked the settings on their end and have it set to send all as .pdf as I was receiving .gif and .tifs. At that point they say it is on my end in the Mail program.

Any ideas - I could not find any settings that made sense to change and cannot recall working with any settings since the issue has started.
