I'm assisting a friend with a 450mhz iMac (about 5 years old) running 10.3.9, 384mgs Ram, with extremely slow performance. I have run Speed Disk, Disk Warrior and Disk Utility. What else could be wrong?

OS X can often be improved by cleaning out the caches, repairing permissions and a few other odds and ends that the disk repair utilities that you have tried so far, really don't touch.
Download a good maintenance utility, such as Onyx, or YASU, or Maintain, or System Optimizer X, or Xupport, or MacJanitor, or any of a couple of dozen others. These can all be searched and downloaded at versiontracker.com
Some are freeware, others are shareware. They ALL do some similar tasks that can help with making a system run a little smoother, and sometimes give a lot of speed back, and is worth doing.

One that I like, YASU (for Yet Another System Utility) does the basic things which would probably help you the most. http://www.jimmitchelldesigns.com/yasu.html There's one for Tiger, and one for Jaguar/Panther, download the one that you need.

I would recommend staying away from Norton SystemWorks with Panther. Symantec stopped development of this software almost two years ago, and running it does nothing of value on Panther, and I have lost files after using it.
On a five year old system, you might also have a hard drive that is starting to get a little fragile and clunky - something to think about - even though a hard drive may test OK, it could be not accessing files like it has in the past. I have replaced a few hard drives in these iMacs, and sometimes makes a big difference. Well, it's a job backing up files, and tranferring to the new drive, and reinstalling the system, but it can improve performance.
