Macbook Goes To Sleep And Doesn't Charge When Power Cord Is

Yosemite OS X
Model: A1466 EMC 2559
1.8GHz dual-core Intel Core i5 (Turbo Boost up to 2.8GHz) with 3MB shared L3 cache MagSafe 2 power port
When I plug in the power cord, the screen instantly goes black and any sound playing turns off (like sleep mode), the power cord does not light up. After I remove it, mac wakes up after 2 seconds. There's a short moment when I plug it that I see the battery icon show that it's charging, however it doesn't actually charge. Changing the power cord or outlet doesn't help. I've been experiencing this for a while now and would usually have to wait either a few minutes or half an hour and the issue goes away. However, this time it's been more than a day and the problem persists. I haven't been able to find anyone else having this problem online and would really appreciate someone shedding some light on what the issue may be and how to fix it. Thank you.

Have you checked the power cord port to make sure it is clear of dust & dirt? Even a little can prevent good contact to charge the battery.

My next thought is that it probably needs a new battery.
