HD name is resetting at start-up

I just formatted my HD and installed a fresh 10.2 (now upgraded to 10.2.6). Now, when I start-up my computer, my main HD name seems to 'reset' itself to a foward-slash "/". So, for example, if I name the HD, "Macintosh HD" and I restart, it shows up as "/" at the next start-up. And it does this every time. Has anyone encountered this before and defeated it?
