Universal Acess' Icon is odd...

Hi, I have a MacBook Pro running the latest OS X 10.4. The icon for Universal Acess is really odd and I wish to change it back to how it was before. You can see picture of it below. I ran disk utility to repair permissions and only have a mouse attached. I don't recall installing anything either. Thanks in advance!

Quit System Preferences if it's running and trash the following file:


/Users/yourusername/Library/Caches/com.apple.preferencepanes.cache

Then log out/in or restart and check System Preferences for proper functionality.
