If I quit Safari before selecting Safari some other App such as LibreOffice or Photoshop Elements 13 will cause the same problem. Also if I try to logout instead of restart I get the same problem. I have tried booting in safe mode and doing a clean install of Yosemite with no benefit.

The app I am thinking of gave you a list with a check box. If you unchecked an item it would not be loaded. However all the items would remain in the list after restarts. With the Login items list in Users all you can do is remove an item from the list.
