I have tried to remove the device (iPhone5) above but there is no way to do that. That is a screenshot of my browser showing the devices that are supposedly connected to my account. It's stuck somewhere and I can't find it.

I gave my iP5 to my daughter and bought a 6. Both were restored and the SIM changed. The old phone still appears in my iCloud/Settings in my browser (Safari). There is no option to delete the 5 and the 5 and her old 4 still show in her iCloud Settings also. This is NOT a Find My iPhone problem...
