Admin password fails

We have been running OSX Server for some time with no problems on an eMac that is used as a mail server. Starting with 10.3 and now 10.4. Recently it has started to loose the Administrators password (ID 501) even though it is logged in under that user. If I try to connect to the server using Server Admin or Workgroup Manager, locally or remotely, it will reject the password. If I connect as another Administrator in Workgroup Manager and re-save the password for the main Administrator (501) it then works fine for a week or two. All other accounts are fine and the Mac is on 24/7 and only re-booted for software updates or lock up's (which is rare). The Mac is set to log in to the main account on boot up. But if it looses the password and you try to re-boot the log in fails. I then have to login as another user and re set the password again.
Anyone have any ideas why this is happening? and how to fix it?
(I have done the usual permission's / disk repair and booted into single user mode to run fsck and all appears to be fine)
