Anyone's "Preview" is looking like that? How do you make it back to normal?

hi,

Recently my "Preview" looked like that.. it was all of a sudden too.
Does anyone know how to change it back? or has gotten the same problem?

The file cant be seen in the (as shown in picture) and the only way to Preview it is by clicking on it, which is a hassle.. and defeats the purpose of Previewing it..

Help, anybody?
