This is the way I've done it ( although there may be a much easier process):
1. You'll need a Mac OS 9 Install CD. Insert it in your cdRom drive.
2. In Applications:Uitilities open DiskCopy.
3. In DiskCopy choose "New Image from device".
4. Choose the device that OS 9 Install is in.
5. Name the file and location and choose read/write for the format.
6. Once it's finished writing the image you can mount it and add the programs you want, I do this to make a bootable Norton Utilities Disk.
7. When you've got everything in you cd image you can go back into DiskCopy and "Burn Image" to write it to disk.
8. Once done boot off the CD you just created.
*You can also do this with Toast but DiskCopy is already installed on your system if you don't have Toast.
Other's please feel free to make any corrections/additions.
