I keep geting the problem of disk write / read error

The disk read /write error continues to appear after lots of ties, I have tried everything. Updating the Itunes to the latest versio, updating the ipod firmware to the latesest versio, I even formated my whole PC and re installed every single thing, and the same problem appears.

What should I do?
