iBook G4 booting and reloading issue

I am having iBook G4 (14") boot problems. When the computer boots up, I hear the chime, then it goes to the Apple and spinner screen. This screen is normally a solid color, but it now has vertical stripes. The Apple logo and the spinner do not have these stripes. The computer will then proceed to the blue screen where I should see the login selection dialog. That dialog does not appear.

Out of frustration, I have tried to reload the OS (10.4). I boot the CD and run the Disk Utility to wipe (write 0s) the disk and repartition. I then try to load. It checks the CD and begins the load process. At some point, it reboots (I haven't had the patience to sit and watch it) without asking for the second CD. The reboot sequence is as described above. After a little while (10 minutes or so), it spits the CD out. It remains at the blue screen.

I have tried to reset the PRAM and reload. Same results. What could be the issue?

Where did the 10.4 Install/Restore CD you're using come from?

Is it a retail version of 10.4? A "Restore" CD that came with the iBook? From another Mac?
 
The Install CD came with the Mac. I bought the machine new 5 years ago.

As an update, I am able to boot into Safe Mode. But, I cannot do most of the things I need under Safe Mode. Once I leave Safe Mode and reboot, it still does not boot properly.
