MacPro DVI KVM problems

I just bought a MacPro and am setting it up with a digital KVM and am having some issues with an occassionally blank screen on reboot or switching the view from one machine to the other. The setup:

Naturally, everything is using DVI connections. The blank screen occurs about two-thirds of the time. When it happens, I can usually switch to a different machine on the KVM, then switch back and everything works. Sometimes I have to go back and forth two or three times.

Has anyone seen a similar issue? Any ideas on fixing it? Is this likely to be a KVM, video card, driver or monitor problem?
 
This video card comes with two DVI outputs. Connecting the KVM to the second instead of the first seems to solve the problem. Not sure what's special about the first one.
