Does anyone know a good script or plug-in for InDesign to create indexes (those extensive tables of contents that usually appear at the ends of books)?

Check Adobe's site, I vaguely remember seeing on around the ID 1.5/CS time on their extensions page.

Yup...but if you do (or a company does) that kind of thing on a regular basis, it's probably worth the money in time saved.
There was another option out there long ago (v.1.5 era). It was either free or a small shareware fee. It could be the precedent to the one being sold now, I'm not sure. It's been so long since I needed something like that for ID.
