USB 4-port PCI Card problem

I have a desktop G3-Power Mac with OS 9.2.2 running. I installed a USB 4-port PCI card (CompUSA) which is compatible with Mac 8.6 and higher OS. However, now I get a system error message and computer freezes.

I tried to restart the computer with Shift ket held down, however, even this approach comes up with the same error message (Turn the Extensions Off).

Could anyone advice me on 1) how to restart without the error message 2) was installing the PCI card a mistake? 3) does any one know, how I can get USB ports on this old G3 desktop.

One thing you might try is OS 9.1 instead of the 9.2.2 persuasion.
If you don't need 9.2's features (mostly hooks to OSX), you don't
need 9.2 at all. Please, folks. OS 9.2 is not an upgrade to 9.1, and
it often doesn't play well with G3s or peripherals.
