duplicates in my apple menu

as you can see on the image, sudden duplicates in my mac menu

360 ?? Ss 0:32.44 /usr/sbin/screenreaderd -w runs no matter what, it generates another process id even though I kill it wil 'kill -9 360'

weird rectangles now and then on the screen, with no fill(transparent) and a black outline.

?!

this is the second time this happends..
 
EDIT: n/m, i just saw the repost....


As for the black rectangles, Sounds like VoiceOver. hit Apple+F5 to turn it off. You can also access by opening System Prefs and clicking on Universal Access.
