"This will open the application for the first time" HUH?

Hey, Look!
Hello

Recently, occasionally when I double click a document an odd alert window will pop up (see attachment). What it says is absolutely not true. This happens with a bunch of apps, some I use all the time. What exactly is this message, what does it mean, and is there a way I can stop it from appearing?

It's part of the security protocol installed recently when people were making a big deal that a hole in Safari could allow for a virus (if one existed) to be automatically opened after downloading.
Even after running cron jobs, or a restart, will you see that message. Just think of it as Apple's anti-virus protection.
 
I just noticed that when I opened a powerpoint file. I thought it was Microsoft being stupid, here it turns out it's Apple.

Unless you run certain cron jobs or dispense with the logs, or do an install such as 10.3.5. Then, the prompts will return.
