Frequently, when I save a file out of one app and then go to open it in another app, it won't show up as being in that folder. Even if I go to the Finder and see the file is there, when I go back to the application and try to Open or otherwise access the file, it doesn't show up.

Eventually of course it does. And I'd say most of the time the delay is modest if any. But sometimes it'll be half a minute or more before the file is "available" in open/save dialogs.

What the heck is going on?

I should note that I DO use DefaultFolder, but I'm wondering if it's just a Unix/OS X thing.

It very well could be Default Folder that's not refreshing correctly. I don't experience that problem, and I don't have Default Folder installed.

Try temporarily disabling it and see what the results are.
 
Seems to not be Default Folder - I turned it off and still the problem.

A little more (and corrected) info. It seems that if I visit a folder in the finder and see that the new file(s) are there, after that they will show up in the open/save dialogs. A lot of times I'll pop over to the finder, watch as the file list grows by one as the new file is "discovered", then go back to the open/save and be able to access it.

The same thing is happening with labels - I have a folder full of files I labelled different colors. I un-labelled them probably 18 hours ago, but when I access the folder in BBEdit they're still colored. Note, though, that I have NOT viewed the folder in the finder - betting once I do the colors will reset.

And then there's the #&^! modification date thing. I don't recall the exact circumstances (seems like most situations), but basically files in folders don't update to reflect new modificaton dates/times until they've been selected. And you can't Command-A on them either - you have to click your way along them. It's pretty lame and one of the biggest failings vs. OS 9 that I'm aware of.

Be nice if Apple could fix this, or someone provide a patch. Even something that would re-scan the foreground window a bit more often would be nice. Sometimes it's hard to call OS X a "step up".
