Macbook fails to start/wake (occasionally)

Here are the problems: When I allow my Mac to go to sleep by closing the lid or selecting Apple -> Sleep, or shut off, I can't seem to wake it.

When it sleeps: When I open the lid, I hear the hard drive spinning up. Often, the display will flash on for a moment, then go black. The system (hard drive) still runs.

When it's shut off: When I shut the computer off, and I start it back up, most of the time, I'll hear the OSX chime, then I'll get the white page with gray apple symbol and the loading symbol. After this, it's supposed to give me the blue background screen and loading bar but it (the display) usually goes black.

For reasons I don't understand, when this problem happens 90% of the time. If I keep on force shutting once I get this black screen, then re-starting, it'll occasionally give me a full startup. No problems in the use afterwards.

I have already run fsck. No errors are reported.

Please help!
 
What shows in console (applications>utilities> console) when that fail to wake/sleep occurs? Console and system logs will show what was just happening.

Do you have any peripherals connected? USB anything? Bluetooth on or off?
