OSX duplicating commands in pull-down menus

OSX has been acting weir the last 2 days. I'm having trouble with some programs (mainly InDesign) SOMETIMES launching properly and sometimes not. Also, after a full day when I shut down my computer form the apple pull down menu, all of the commands (i.e. 'restart', 'log out', etc.) are displayed twice. That has never happened before. I also tried to open a folder that was in the trash so I could just pull one file back to my desktop and everything on my screen disappeared aside from the dock (luckily). I tried a restart and the words 'restart' were missing from the face of the blue button that verifies your reuqest.
Anybody seen this before? What's going on and how can I fix it? Thanks a bunch.
 
It is always off. There were a few occasions when I accidentally activated it with the short keys the last few days, but I would immediately turn it off in system preferences once I heard anything. Could it have been just because voice over was activated earlier that day? That doesn't really make much sense to me. Please explain if this is the case. Thanks.
 
So, is VoiceOver on or off when the double menu items appear?

You can disable the keyboard shortcut for VoiceOver if you don't use it in the "Keyboard & Mouse" pane of the System Preferences under "Keyboard Shortcuts" if you find you accidentally activate it frequently.
 
yeah, voiceover was off when i shut down which i when i noticed the duplications within the apple pull-down menu. I have since disabled the hot key activation, so hopefully I won't see that anymore. Is voiceover the only reason why that particular issue would arise?
