software to create interactive form

I am a new mac user with the basic software including iwork08. I would like to create a registration form for workshop participants to complete online and to capture info in some useful format but will settle for just printing out the completed form. Any suggestions?

Just to elaborate, Adobe LiveCycle is a tool used to create PDF forms that have editable fields, just like you want. It is included with Adobe Acrobat 8.0 Professional, available from http://www.adobe.com. The end-result would probably be a link on your website to a downloadable PDF file, which you could then instruct users to email to you.

Creating an online form to capture information may be more difficult than you think -- you can have the results emailed to you, but they won't be in any kind of fancy format unless you put some work into it. If you want the results stored somewhere, that would involve setting up some sort of database as well... in short, you'd probably need to be a web designer/web programmer and skilled in creating interactive websites in order to produce what you want for online use.

There are some skilled web designers on this site who would probably be willing to help you out (including myself!).
